Privacy Policy - the basics
Having said that, a privacy policy is a statement that discloses some or all of the ways a website collects, uses, discloses, processes, and manages the data of its visitors and customers. It usually also includes a statement regarding the website’s commitment to protecting its visitors’ or customers’ privacy, and an explanation about the different mechanisms the website is implementing in order to protect privacy.
Different jurisdictions have different legal obligations of what must be included in a Privacy Policy. You are responsible to make sure you are following the relevant legislation to your activities and location.
What to include in the Privacy Policy
Generally speaking, a Privacy Policy often addresses these types of issues: the types of information the website is collecting and the manner in which it collects the data; an explanation about why is the website collecting these types of information; what are the website’s practices on sharing the information with third parties; ways in which your visitors and customers can exercise their rights according to the relevant privacy legislation; the specific practices regarding minors’ data collection; and much, much more.
Booking Policy
1. Cancellation Policy
Clients must cancel or reschedule appointments at least 24 hours in advance. Appointments canceled with less than 24 hours’ notice may be subject to a cancellation fee.
2. No-Show Policy
If a client fails to arrive for their scheduled appointment without prior notice, service fees may be forfeited and a new appointment will require an additional payment.
3. Refund Policy
All service fees are non-refundable once the screening, testing, or fingerprinting process has begun. Refunds for prepaid services will only be issued if ProVital Screening Solutions cancels the appointment.
4. Mobile Service Policy
For mobile appointments, the client must be present at the scheduled time and location. If the technician arrives and the client is unavailable, a mobile trip fee will still apply. Additional fees may be charged for travel outside of the standard service radius.
5. Late Policy
Please arrive on time for your appointment. Clients arriving more than 10 minutes late may be asked to reschedule depending on availability.
6. Privacy & Confidentiality Policy
All personal information, test results, and client records are kept strictly confidential and secure. ProVital Screening Solutions complies with all federal and state privacy laws, including HIPAA and DOT regulations when applicable.
7. ID Requirements
A valid, government-issued photo ID is required for all testing, fingerprinting, and verification services. Failure to present proper identification may result in appointment cancellation.
8. Payment Policy
Payment is due at the time of booking or prior to the service being performed. We accept all major payment methods, including mobile payments and invoicing for business clients.
9. Turnaround Time Policy
Most negative drug test results are available same-day or within 24 hours. Background checks and employment verifications may vary depending on the employer, agency, or state processing time.
